To add a public form page, click the New button. To delete a from, select the row by clicking anywhere to the right of the form name and then click the Delete button. To modify a form, click on the form name.
The Form URL column shows two URLs. The first uses the encoded Form Name as a parameter to the URL (note that forms can be renamed which will change this URL). The second uses an internal identifier for the form that is never changed or reused even if a form is renamed or deleted and recreated.
When publishing URLs for public forms it is recommended that the PERMALINK be used.
